Report Type	Monthly or Grouping
Group By	No Grouping Customer Test Lab Tech
Date From / To	Select the Date from and to - Default 6 months back from current date
Customer From / To	When grouping by customer is active, select customer from and to or leave blank for all
Status	Choose a status to print
Report By	Number of Tests Value of Invoiced Tests
Test Group	Seed Tests (this cannot change)
