Commission	to select commission from the drop down arrow
Date From	to select the date from which the report will be running from
Sort By	to sort the report by selecting from the drop down arrow
Rep Type
Report Type	to select the type of the report from the drop down arrow
Date To	to limit the report by entering the date to
Show Debtor Code
Show Costs
