Code	Enter Website Support Form Code
Description	Enter Website Support Form Code description
Sequence No	Enter in which order the code is to appear
Group	Select from drop down to which Website Support Group you want to associate this Support Form to
Attachment	You are able to attach a document or picture to the Support Form
Active	Searching for Active or Inactive Support Forms
