 This function allows you to Add/Edit or Deleted the Website Support Form

Code	Enter Website Support Form Code
Description:Description	Enter Website Support Form Code description
Sequence No:No	Enter in which order the code is to appear
Group:Group	Select from drop down to which Website Support Group you want to associate this Support Form to
Attachment:Attachment	You are able to attach a document or picture to the Support Form
Active:Active	Searching for Active or Inactive Support Forms

Save & Close button: saves the contents of the for and return you to the Support Form Search window

Save button: saves the contents of the form

Close button: closes the form and returns you to the Support Form Search window
